• Enter your First, Last Name, email address and phone number that you used to join or renew your membership.
• Enter the last 3 or 4 digits of your keycard(s) as shown below in the red circle on the form for card number 1.
• Enter the last 3 or 4 digits of your keycard(s) as shown below in the red circle on the form for card number 2, if you have a second card.
• NOTE: Older keycards will have 3 digits.
